Papercopies of the Flu Vaccination Consent Form
We will be sending out papercopies of the Flu Vaccination Consent Form today to anyone who did not receive a flu vaccination yesterday (Reception-Y6 only)
If you OPTED "NO" online - you will not receive another form.
If you did not know how to opt, forgot or did not know the vaccination was taking place - then these forms are for you :-) Please OPT "YES" or "NO" on the papercopy as soon as possible and return the form to school and the Nurses will hopefully be back before Christmas for a mop up session.
If you OPTED "YES" online, but are sure you child did not get the vaccination then please call us and we can check the Nurses registers.
